Qualifications

Under the direction of local leadership, the Oncology Nurse Navigator (ONN) guides the patient's journey from pre-diagnosis through treatment. Collaborating with a multidisciplinary team including PCPs, surgeons, GYN, GI, oncologists, and others, the ONN supports oncology patients by facilitating access and timely care. The ONN makes referrals as needed (e.g., financial counseling, nutrition, social work).

This Nurse Navigator position is based at the John Theurer Cancer Center @ Jersey Shore University Medical Center. It involves 4 ten-hour shifts per week, with no weekends. Preference is given to candidates with 2 years of Oncology/Leukemia/BMT experience.

Responsibilities
  • Coordinate outpatient care for oncology patients, supporting them from pre-diagnosis through survivorship or end of life.
  • Build and maintain relationships with medical oncologists, radiation oncologists, surgeons, and other healthcare professionals involved in patient care.
  • Communicate effectively with physicians and staff to outline navigation programs and resources, acting as a resource for patients and families from initial consultation through treatment.
  • Ensure timely and comprehensive information exchange among team members.
  • Facilitate access to services, track patient flow, document assessments and follow-ups in the medical record, and report activity monthly.
  • Assess patient needs continuously, including educational, financial, psychosocial, nutritional, and clinical, referring patients to appropriate resources.
  • Collaborate with the research department and participate in Tumor Board preparations.
  • Develop survivorship care plans for eligible patients, discussing and distributing them appropriately.
  • Engage in performance improvement activities, develop guidelines, and participate in department committees.
  • Represent patient navigation services to the community, working with marketing and outreach teams to promote awareness and connect patients to community resources.
  • Perform other duties and projects as assigned, adhering to organizational standards.
Qualifications
  • Bachelor of Science in Nursing (BSN) required for hires on or after March 2024.
  • Minimum of two years of nursing experience.
  • Excellent communication skills, both written and verbal.
  • Proficiency in computer skills, including Microsoft Office and Google Suite.
Preferred Qualifications
  • Oncology experience preferred but not required.
  • Oncology Certified Nurse (OCN) certification preferred.
Licenses and Certifications
  • New Jersey State Professional Registered Nurse License.
  • Oncology Certified Nurse (preferred).
