Nurse Navigator - Bone Marrow Transplant

Position Details

  • Location: John Theurer Cancer Center @ Jersey Shore University Medical Center
  • Schedule: 4 10-hour shifts per week, no weekends
  • Experience: 2 years of Oncology/Leukemia/BMT preferred

Responsibilities

  • Coordinate outpatient care for oncology patients, from pre-diagnosis through survivorship or end of life.
  • Build relationships with the multidisciplinary team involved in patient care.
  • Communicate with physicians and staff to outline navigation programs and resources.
  • Facilitate access to services, track patient flow, and document in medical records.
  • Assess patient needs and connect them to educational, financial, psychosocial, nutritional, and clinical resources.
  • Participate in tumor board meetings and performance improvement activities.
  • Develop survivorship care plans and ensure compliance with regulatory standards.
  • Represent patient navigation services in community outreach and marketing efforts.

Qualifications

  • Bachelor of Science in Nursing (BSN) required for hires on or after March 2024.
  • Minimum of two years of nursing experience.
  • Excellent communication skills.
  • Proficient computer skills (Microsoft Office, Google Suite).

Licenses and Certifications

  • NJ State Professional Registered Nurse License.
  • Oncology Certified Nurse preferred.
