Job summary

Cancer Center of South Florida PLLC is seeking a Nurse Navigator to coordinate oncology care across inpatient and outpatient settings. You will serve as a liaison among patients, families, physicians, and the care team, ensuring smooth transitions and informed decision-making throughout the cancer journey.

The role requires a BSN, at least 2 years of oncology nursing experience, and an RN license in the state.

Qualifications

• BSN required.
• Minimum of 2 years of oncology nursing experience.
• RN license in the applicable state (required).
• OCN certification preferred.
• BLS certification required.

Responsibilities

• Serve as the primary liaison for newly diagnosed oncology patients and families.
• Improve care coordination by managing services across diagnostics, treatment, and transitions.
• Prepare patient charts and maintain accurate EMR documentation.
• Guide patients through intake, admissions, and discharge planning.
• Act as clinical resource for staff and administration regarding navigated patients.
• Assist with paperwork, referrals, and access to supportive resources.
• Explain procedures and timelines to patients and communities to promote understanding.
• Stay current on oncology services, research, and community resources.
