Munson Healthcare Careers in Petoskey, MI is seeking a licensed RN for a full-time day-shift role (Mon-Fri, 8am-4:30pm) in the Petoskey Specialty Clinic. The position focuses on care management and coordination for patients with complex needs across pediatric and adult populations.

The ideal candidate has Michigan RN licensure, experience with chronic disease management, a BSN, and several years of RN practice. Vaccination requirements apply as part of employment with Munson Healthcare.

• Licensed RN in the State of Michigan.
• Experience in complex chronic disease management preferred.
• Bachelor of Science degree in nursing preferred.
• Five years' (5) experience as an RN preferred.

Responsabilidades

• Act as a liaison between patients, professional staff and physicians by providing care management and coordination for adult and pediatric patients with complex conditions, social needs and educational needs.
• May work on the clinical side if needed.
• Serves in an expanded health care role to collaborate with primary care providers, specialists, and patient/families to ensure the delivery of quality and cost-effective health care services.
• Assesses plans, implements, coordinates, monitors and evaluates all options and services with the goal of optimizing the patient's health status and decreasing hospital and emergency room utilization.
• Support quality improvement initiatives and adherence to safety standards.
