Job summary

Mercy Hospital South is seeking a Nurse Navigator for Cardiac EP in an outpatient clinic setting. You will guide EP patients and families through evaluation, treatment, and follow-up, ensuring a seamless experience and strong collaboration with care teams.

You will coordinate evidence-based care plans, document in EPIC, and drive quality improvements while supporting program growth and community awareness.

Qualifications

• Graduate from approved school of nursing.
• Current RN license in state of practice.
• BLS certification required; ACLS preferred.
• 2–3 years' experience in outpatient or cardiac care.
• Strong communication with physicians and care teams.

Responsibilities

• Guide EP patients and families through evaluation, treatment, and follow-up.
• Coordinate individualized, evidence-based care plans.
• Serve as a clinical resource for physicians and teams.
• Identify, track, and navigate patients using diagnostic criteria.
• Document in EPIC and utilization systems for data-driven decisions.
• Drive quality improvement initiatives and program growth.
• Build relationships with referring providers and community partners.

• Guides EP patients and families through evaluation, treatment, and follow-up, ensuring a seamless outpatient experience.
• Coordinates individualized, evidence-based care plans to optimize outcomes and improve patient satisfaction.
• Serves as a trusted clinical resource for physicians and care teams, supporting best practices and innovative EP care.
• Identifies, tracks, and navigates patients using diagnostic criteria; monitors outcomes and quality metrics.
• Drives quality improvement initiatives that enhance care delivery, efficiency, and resource utilization.
• Accurately documents in EPIC and utilization systems to support data-driven decision-making.
• Collaborates with physician leaders, program partners, and industry representatives to support program growth and innovation.
• Builds strong relationships with referring providers and supports community and program awareness.
• Embraces professional growth, teamwork, and a culture of continuous improvement.

Education: Graduate from approved school of nursing

Licensure: Current RN (Registered Nurse) license in state of practice.

Experience: 2-3 years' experience required.

Certifications: BLS (Basic Life Support)

Physical Requirements:

• Position requires individuals to push, pull, and/or lift 50 pounds regularly.
• Position requires prolonged standing and walking during each shift.
• Position requires the ability to grip, reach, bend, kneel, twist, and squat to perform duties

Preferred Education:

• Bachelor of Science in Nursing preferred.

Preferred Certification

• ACLS Advanced Cardiac Life Support
