Job summary

Sarah Bush Lincoln’s Cancer Center seeks a Nurse Navigator to coordinate care from diagnosis through survivorship, meeting patients and families to provide education and emotional support. This role serves as a single point of contact for medical information and resources.

Hours are Mon–Thu 8am–4:30pm and Fri 8am–3pm with starting pay of $35.43 per hour, based on experience, at the Cancer Center location.

Qualifications

• ADN required.
• BLS or CPR certification within 90 days of hire.
• RN license in Illinois.

Responsibilities

• Educate patient and family; document teaching and assessments.
• Relay information among providers, patients, families, pharmacies, nursing homes, and staff with accurate documentation.
• Schedule diagnostic, therapeutic, referral, or additional office visits.
• Support community educational events and end-of-life coordination.

Nurse Navigator

Job Description

The Nurse Navigator serves as a consistent care coordinator ensuring timely access and seamless coordination of services throughout the continuum of care, from diagnosis through survivorship. The Nurse Navigator will meet with the patient and their support systems to provide emotional support and guidance. As a single point of contact, the Nurse Navigator will provide medical information, education, and share knowledge regarding internal and external support services and resources.

Monday-Thursday 8am-4:30pm

Friday 8am-3pm

Required: ADN, RN License State of IL, BLS or CPR within 90 days of hire

Pay: Based on experience, starting at $35.43

Location: Cancer Center

Responsibilities

Provides education and information to the patient and family. Initiates and documents patient teaching, including family and significant others, based on assessment of needs., Relays information between provider(s) and client(s), which may include but is not limited to communications with patients, families, pharmacies, nursing homes, office staff, insurers, and facilities. At provider’s instruction, communicates provider advice, instructions, orders and results, as applicable, including accurate documentation of exchanges., Schedules patients for diagnostic, therapeutic, referral, or additional office visits., Serves as a resource for community educational events, such as health fairs, screenings, symposiums, and lectures., Supports the patient during difficult decision-making periods. Assists in coordination of end-of-life care for patient and provides emotional support.
