Job summary

Hartford HealthCare is seeking a Clinical Nurse Manager to lead the outpatient Transplant Department, ensuring safe, high-quality, patient-centered care across the transplant continuum. You will partner with physicians, coordinators and the care team to optimize outcomes and efficiency.

Qualifications

  • Current Connecticut Registered Nurse license required.
  • Bachelor's degree required; Master's degree in Health Administration, Nursing Leadership, or related preferred.
  • Specialty Certification in transplantation, ambulatory care or nursing leadership preferred.
  • Minimum 3 years relevant management experience.
  • CPR certification preferred.
  • Five or more years of progressive nursing experience, ambulatory or outpatient clinical practice.
  • Strong written and verbal communications skills required.
  • Exceptions to the educational requirement will be evaluated on an individual basis with progress toward completion within five years.

Responsibilities

  • Direct and oversee daily operations of the outpatient transplant clinic to ensure safe, efficient, patient-centered care.
  • Coordinate clinical workflows across pre-transplant evaluation, post-transplant follow-up and specialty services.
  • Monitor patient throughput, clinic capacity, access, scheduling efficiency and resource utilization.
  • Develop and implement standardized clinical workflows and processes to improve quality and productivity.
  • Collaborate with physicians, transplant coordinators, APNs, pharmacy, social work and other departments for seamless patient care.
  • Participate in leadership planning for program expansion and process improvements.

Job Summary

The Clinical Nurse Manager is responsible for the clinical and operational leadership of the outpatient Transplant Department. This role provides direct oversight of daily operations and ensures the delivery of safe, high-quality, patient-centered care across the transplant continuum. The Clinical Nurse Manager partners with physicians, advanced practice providers, transplant coordinators, multidisciplinary team members and organizational leadership to optimize patient outcomes and improve operational efficiency. The Clinical Nurse Manager promotes a culture of excellence, accountability, collaboration and continuous improvement while supporting strategic initiatives and organizational goals.

Key Areas of Responsibility
  • Direct and oversee the daily operations of the outpatient transplant clinic to ensure safe, efficient and patient centered care.
  • Coordinate clinical workflows across pre-transplant evaluation, post-transplant follow-up and specialty transplant services.
  • Monitor patient throughput, clinic capacity, access, scheduling efficiency and resource utilization.
  • Develop and implement standardized clinical workflows and operational processes to improve quality, patient experience and productivity.
  • Collaborate with physicians, transplant coordinators, advanced practice providers, pharmacy, social work, RD and ancillary departments to ensure seamless patient care.
  • Participate in operational planning for new programs, service expansion and process improvement initiatives.
Leadership
  • Recruit, hire, onboard, mentor, develop and retain nursing and clinical support staff.
  • Provide ongoing coaching, performance evaluations, competency assessments and professional development opportunities.
  • Foster employee engagement, accountability, teamwork and a culture of continuous learning.
  • Develop staffing plans and manage scheduling to ensure appropriate coverage while maintaining fiscal responsibility.
  • Participate in leadership meetings to evaluate program performance and operational priorities.
Patient Experience
  • Promote exceptional patient and family experiences throughout the transplant journey.
  • Resolve patient concerns and service recovery issues promptly and professionally.
Qualifications

  • Exceptions to the educational requirement will be evaluated on an individual basis. For example, a strong candidate in the process of completing the educational requirement will be considered. Continued progress toward completion of the educational requirement within five years will be required.
