Job summary

Gurwin Healthcare System is seeking a Nurse Manager, RN to lead admission assessments and oversee comprehensive nursing care for residents across a full continuum of care on our 36-acre Commack, NY campus. You will help formulate and implement patient care plans, supervise staff, and coordinate with rehabilitation teams to deliver high-quality geriatric care.

The role requires graduation from an accredited RN program with NY licensure; a BSN is preferred and a current license is required.

Qualifications

• Graduate of an accredited professional R.N. nursing program with a current license to practice in New York State.
• BSN preferred.
• Current license required.

Responsibilities

• Formulates and implements patient care plans for geriatric residents.
• Ensures nursing diagnoses are based on physical and psycho-social observation.
• Directs rehabilitation and restorative nursing and liaises with Rehabilitation services.
• Supervises nursing staff, coordinates admissions/re-admissions, and ensures policy adherence.
• Participates in interdisciplinary conferences and unit management planning.
• Performs related duties and responds flexibly in emergencies.
