Job summary

Gurwin Healthcare System in Commack, NY is seeking a Nurse Manager, RN to lead admission assessments and all components for new admissions and readmissions. The role requires NY RN license, BSN preferred, and leadership in geriatric patient care within a full continuum setting.

Qualifications

• Graduate of an accredited professional RN nursing program with a current license to practice in New York State.
• BSN preferred.
• Current license required.

Responsibilities

• Formulation and implementation of patient care plans for geriatric patients using nursing assessment and diagnostic skills.
• Ensure nursing diagnoses for residents are based on physical and psycho-social observations.
• Monitor changes in resident status and ensure timely responses by unit staff.
• Direct application of rehab/restorative nursing and liaison with Rehabilitation.
• Develop and supervise nursing approaches for resident care (e.g., nursing orders).
• Assist in direction, supervision, instruction, and evaluation of other nursing personnel as assigned.
• Observe, record, and report patient data verbally and in writing per policy.
• Participate in unit nursing and inter-disciplinary conferences; assist in planning unit management with Nursing Care Coordinator.
• Ensure adherence to institutional philosophy and policy in professional practice.
• Attend in-service programs and participate in committees as assigned.
• Perform related duties; in emergencies, may perform unrelated tasks.
• Remain flexible to dynamic clinical environments.

Set on a 36-acre campus in Commack, NY, the Gurwin Healthcare System includes the 460-bed Gurwin Jewish Nursing & Rehabilitation Center, providing short-term rehabilitation and long-term skilled nursing care, memory care, respiratory and ventilator care, palliative and hospice care, social and medical adult day programs, and on-site dialysis and infusion therapy. Sharing the campus is Gurwin’s Assisted Living community (Gurwin Jewish : Fay J. Lindner Residences) and our independent living community (Fountaingate Gardens).

Responsibilities

• Assumes responsibilities for the formulation and implementation of patient care plans to meet the specialized needs of geriatric patients using nursing assessment and diagnostic skills.
• Assures that nursing diagnosis for every resident is based on physical and psycho-social observation (self-care deficit needs).
• Assures for every resident; changes in resident status and appropriate response by unit staff are continuous and timely.
• Directs the application of knowledge in rehab/restorative nursing and liaison with Rehabilitation.
• Develops and supervises nursing approaches for resident care (ie: nursing orders)
• Assists in direction, supervision, instruction, and evaluation of other nursing personnel as assigned.
• Observes, records, and reports patient data both verbally and in writing as dictated by departmental policy.
• Participates in unit nursing and inter-disciplinary conferences. Assumes a role in planning unit management with Nursing Care Coordinator.
• Assumes responsibility for adherence and support of the institutional and departmentalphilosophy and policy in professional practice.
• Attends nursing and other institutional in-service programs and participates in departmental and institutional committees as assigned.
• Performs related and miscellaneous duties as directed. In an emergency situation, may be expected to perform unrelated duties.
• Ability to respond flexibly to a dynamic clinical environment.

Requirements
