Job summary

Nuclear Care Partners is seeking a Nurse Manager in Grand Junction, CO to lead a home care team with hands-on field support and strong clinical oversight. This exempt role emphasizes compassionate, high-quality care and strategic leadership to benefit patients and families.

The Nurse Manager will supervise clinical staff, ensure compliance with state and federal standards, and partner with physicians and other providers to coordinate care. Excellent nursing leadership is essential.

Qualifications

• Bachelor of Science in Nursing required or preferred.
• Registered nurse license in good standing required.
• Experience in case management or progressive healthcare management preferred.

Responsibilities

• Assist with supervising activities to promote safe and compassionate nursing care and patient satisfaction.
• Participate in initial assessments and start of care recerts.
• Oversight, management, and support of field staff trainings and orientations.
• Ensure policies, procedures, and regulations are followed.
• Provide direct case management to assigned caseload.
• Manage skilled services and coordinate clinical team scheduling, training, and coaching.
• Maintain thorough documentation and compliance with state and agency standards.
• Availability for in-person or phone support during operating hours.
• Support payroll charting and chart clearing for assigned staff.
• Represent client issues with staff and external providers.
• Monitor RN utilization, education hours, and renewal appointments.
