Job summary

Licensed Practical Nurse – Phoenix House

Responsibilities
  • Provide direct nursing care to clients under the supervision and direction of clinical leadership.
  • Maintain patient confidentiality in accordance with Phoenix House policies and procedures.
  • Oversee unlicensed assistive personnel (e.g., CNAs, medical assistants, and medical technicians).
  • Perform routine nursing procedures such as EKGs, wound care, and specimen collection. Administer prescribed medications in accordance with NYS scope of practice and facility policies.
  • Document vital signs, weight, and other clinical assessments to monitor patient condition and report significant findings to clinical leadership.
  • Observe and promptly report any changes in patient behavior, appearance, or health status to appropriate clinical leadership.
  • Engage in health education and reinforce treatment plans, medication adherence, and self‑care strategies.
  • Other duties as assigned.
Qualifications
Required Education
  • High School Diploma or GED
  • Graduate of an accredited nursing program
Licenses/Certifications
  • Current NYS Licensed Practical Nurse
  • First Aid Training
  • CPR Training
  • Narcan Training
  • Infection Control Certification
  • Child Abuse Certification
Required Experience
  • At least three years of related experience, qualified by training, education, and experience in providing nursing care for chemically addicted patients and patients withdrawing from alcohol and/or drugs.
