Job summary

Phoenix House of New York is seeking a Licensed Practical Nurse to support clients in a Long Island City residential setting. You will provide nursing care, assist with medication support, monitor vital signs, and coordinate care with the treatment team to ensure continuous, compassionate support for clients in recovery.

Licensed Practical Nurse Will Perform the Following Tasks

• Provide direct nursing care to clients under the supervision and direction of clinical leadership.
• Maintain patient confidentiality in accordance with Phoenix House policies and procedures.
• Oversee unlicensed assistive personnel (e.g., CNAs, Medical Assistants, and Medical Technicians).
• Perform routine nursing procedures such as EKGs, wound care, and specimen collection. Administer prescribed medications, including oral, topical, and injectable forms (IM, Sub-Q, intradermal) in accordance with NYS scope of practice and facility policies.
• Document vital signs, weight, and other clinical assessments to monitor patient condition and report significant findings to clinical leadership.
• Observe and promptly report any changes in patient behavior, appearance, or health status to the appropriate clinical leadership.
• Engage in health education and reinforce treatment plans, medication adherence, and self-care strategies.
• Other duties as assigned.
