Job summary

You will ensure assessment, care planning, and documentation while maintaining compliance and compassionate engagement with residents and families.

Qualifications

• LPN or RN license required.
• Minimum 2 years in geriatric nursing.
• 1 year in supervisory/management position.
• Ability to read, write, speak and understand English.
• Current First Aid and CPR.
• Pass a Criminal Background and fingerprint check.
• Weekend availability.

Responsibilities

• Foster resident-centered planning and service delivery.
• Assess residents’ health and psychosocial status; initiate service plans.
• Coordinate with physicians, nurses, home health staff, pharmacies.
• Delegate nursing tasks and communicate with families and health care providers.
• Maintain resident care records and ensure policy compliance.
• Provide staff training, supervision, and monitoring of care.
