Responsibilities

• Explain services available and engage client to access services.
• Completes health screenings and facilitates outside referrals as directed by care team.
• Participates in team meetings to develop a plan of care for clients.
• Assists clients in navigating the healthcare environment for access to care.
• Advocates for clients with healthcare providers and managed care organizations.
• Assist with transportation options to appointments when needed.
• Identify other needs to ensure access to healthcare.
• Assist in housing access if housing destabilizes health status.
• Assist with access to community resources such as public assistance and financial benefits.
• Develop knowledge of local healthcare providers and collaborate as needed.
• Document tracking for appointments, medications, and other needs.
