Job summary

The Texas Health and Human Services Commission seeks a PNMT Nurse (RN) at the El Paso State Supported Living Center. This full-time role involves evaluating assessment data, providing PNMT recommendations, and collaborating with the Interdisciplinary Team to integrate physical and nutritional management into Individual Support Plans.

Qualifications include active Texas RN licensure, 1–2+ years of RN experience, and preferably a Bachelor’s degree.

Qualifications

• Active RN license in Texas is required.
• Minimum 1 year RN experience; 2+ years preferred.
• Bachelor's degree preferred in nursing or related field.
• Experience with PNMT policy and ISPs is a plus.

Responsibilities

• Provide PNMT nursing assessments and develop measurable goals and action plans.
• Collaborate with IDT to integrate PNMT recommendations into ISP.
• Conduct PNMP reviews post-hospitalization and document findings.
• Maintain documentation and assist with PNMT policy development.
