Job summary

The Texas Health and Human Services Commission (HHSC) Richmond State Supported Living Center is seeking a Lead PNMP Coordinator to oversee physical and nutritional management plans and mentor staff. You will train direct support professionals, ensure proper implementation of PNMPs, and maintain accurate client records.

Qualifications

• High School diploma, GED, or 6 months of work experience required.
• Experience with people who have mental health/ID disabilities preferred.
• Knowledge of PNMP services and rehabilitation programs preferred.
• Experience writing reports is preferred.

Responsibilities

• Lead PNMP team activities and coordinate PNMP services.
• Train and mentor direct support staff in PNMP procedures.
• Maintain PNMP documentation and client records.
• Monitor PNMP program quality and ensure staff compliance.
• Interact with Habilitation Therapists to align plans.
