Job summary

Northeast Missouri Health Council's Envision Wellness team seeks a qualified professional to provide evidence-based diabetes self-management education and support (DSMES), nursing assessment, and chronic disease education. You will coordinate care, support diabetes technologies, and engage in community wellness initiatives.

The role emphasizes patient outreach, collaboration with healthcare professionals, and contributions to quality improvement and program development.

Qualifications

• Experience delivering DSMES per accepted guidelines.

Responsibilities

• Develop individualized care plans for patients.

The qualified candidate will join the Envision Wellness interdisciplinary team and provide evidence-based diabetes self-management education and support (DSMES), nursing assessment, chronic disease education, care coordination, and population health services. Responsibilities include developing individualized care plans; providing diabetes education; supporting CGM, insulin pumps, and other diabetes technologies; coordinating care with providers and other healthcare professionals; conducting patient outreach and follow-up; and participating in community wellness, health screenings, quality improvement, and program development. Candidate must provide excellent patient care, communicate effectively, work independently and collaboratively, and have strong computer/EMR skills.
