Responsibilities

• Collaborate with the interdisciplinary team to coordinate care.
• Assess residents and develop programs to maximize abilities and well-being.
• Perform quality improvement evaluations and report issues for resolution.
• Create daily and monthly activity calendars and implement activities.
• Advocate for residents and families through education and support.
• Manage admissions and discharges from the unit.
• Monitor the Dementia Program unit budget and financial performance.

Responsibilities

• Member of the of nursing center interdisciplinary team and as such participates in staff meetings to ensure coordination of quality patient/resident care.
• Assess each patient/resident and develop programming that maximizes the customer's abilities and provides contentment, enjoyment, and positive self-esteem.
• Perform ongoing quality improvement evaluations that include quality indicators for the environment, documentation, team functioning, and all aspects of quality care provision as well as reporting identified issues back to the team for resolution.
• Develop daily and monthly activity calendar for the unit that includes both small and large group activities and participates in their implementation.
• Advocate for patients/resident and their families and provides direct services to families through communication, education, and support.
• Manage all patient/resident admissions and discharges from the unit.
• Oversee the financial performance of the Dementia Program unit by developing and monitoring budget.

Qualifications

• Must be a graduate of an accredited college or university with a Bachelor's or Master's degree in social work, nursing, therapeutic recreation, psychology, gerontology, or other related field preferred.
• Three years of direct experience (in respective discipline) with dementia/patients who have dementia, and/or their families and caregivers, or approval by the Nursing Home Administrator/Executive Director.
• Two years of supervisory experience in an appropriate health care setting, or approval by the Nursing Home Administrator/Executive Director.
• Recommended state licensure in respective discipline is required.
• Must meet state requirements for concurrent job responsibilities, if applicable.

Posted Salary Range
