Description

This position serves in an expanded nursing role working in collaboration with the division Family Medicine providers that provide care to patients in the outpatient clinic setting. The positions main focus will be supporting the care team in Kids Health Harbor Foster Care clinic and Child abuse clinic with support to other primary clinics as well. This position will coordinate care and provide support to the providers to ensure a care model that is conducive to the delivery of quality, efficient, and cost-effective healthcare services. Primary focus is on pediatric patients, collaboration with foster care parents, DCFS caseworkers and the Child Advocacy Center. This position involves a high level of interaction, coordination and education.

Examples of Duties
Direct Patient Care 80%
  • Works under the direction of the Faculty providers at Center for Family Medicine to provide nursing support and assist with scheduling patients in Kids Health Harbor and Child Abuse clinic.
  • Serves as an extension of the Faculty and Resident providers at CFM, establishing rapport with the patient and/or caregivers, community agencies, DCFS caseworkers and patient advocates to ensure quality of care.
  • Provides patient and caregiver education as necessary and communicate care needs with the provider and care team.
  • May attend monthly meetings with faculty members at CFM that provide care to patients in Kids Health Harbor and Child abuse clinic to gain understanding of and participate in the patient's care and treatment.

Assists team in proactively identifying care needs or linking with applicable services to facilitate quality of care. 5) Within the health care team, identifies patients at higher risk for care gaps, complications and assist with coordination efforts to break down barriers and implements a patient-centered, nursing plan of care that promote health improvements

Administrative/Teaching 20%
  • Provides annual and monthly orientation for residents and students regarding care of pediatric patients. Assist with annual education surrounding school physical and pediatric well child care.
  • Assists in collection and analysis of data for quality improvement projects.
  • Keep accurate records for patients evaluated for child abuse and ensure documentation is accurate and completed in a timely manner.
  • Participates on internal quality improvement teams
  • Participates on quality improvement committees in the Center as well as externally through the school and at the hospital, as needed.
  • Works with the health care team to continuously evaluate processes, identify problems and propose improvement strategies to enhance deliver of care model.
Qualifications
Credentials to be Verified by Placement Officer
  • Licensed as a registered professional nurse in the State of Illinois (If an Illinois Resident is licensed as a professional nurse in a state other than Illinois, the applicant must meet the criteria established by the Illinois Department of Financial and Professional Regulation to obtain the proper licensure within five (5) months of the date of appointment.)
  • Bachelor's degree
  • Two years of public health or specialized nursing experience
Knowledge, Skills & Abilities (KSA’s)
  • Thorough knowledge of public health nursing
  • Communication (written and verbal) and interpersonal skills
  • Ability to work independently.
