Job summary

The Department of Veterans Affairs in Omaha, NE seeks a Community Care Coordinator (Registered Nurse) to guide Veterans through care options, coordinate referrals, and work with VA and community providers.

You will triage requests, organize care, and support timely access to high-quality care while maintaining compliance with VA processes. Requires RN licensure, English proficiency, and eligibility for bridge program if needed.

Qualifications

• English language proficiency required for direct patient care.
• Graduate of an nursing program approved by ACEN or CCNE.
• Current, full, active, unrestricted RN license in a US state or territory.

Responsibilities

• Assess, organize, facilitate, and guide Veterans through their full range of care options including internal/direct care.
• Serve as liaison to internal and community providers and manage Veterans care through the consult process.
• Coordinate referrals and scheduling into community care and ensure timely, high-quality care.
• Comply with VA health information systems to document, exchange, and track care coordination information.
