Transitions LifeCare in North Carolina seeks a full-time RN supervisor to coordinate nursing staff and ensure continuity of care for hospice patients. You will oversee the interdisciplinary team, manage scheduling, and drive quality improvements while ensuring regulatory compliance.

The role requires strong leadership, supervisory experience, and a solid hospice/home health background. Travel to TL offices across NC and on-call responsibilities are expected part of the schedule.

Qualifications

  • RN with current NC license.
  • Two years supervisory experience preferred.
  • 5 years recent clinical experience with hospice/home health.
  • Thorough understanding of hospice interventions and regulatory standards.
  • Excellent written and verbal communication skills.
  • Must have access to transportation and travel to NC offices.

Responsibilities

  • Coordinate interviewing/hiring of new clinical staff as assigned.
  • Supervise and coordinate delivery of patient care on a 24/7 basis.
  • Coordinate and problem-solve patient care issues with families, physicians and staff.
  • Ensure complete, timely documentation per agency policies.
  • Manage personnel actions including hiring, orientation, performance appraisals and scheduling; counsel and discipline staff.
  • Develop and implement processes to improve clinical operations and quality.
  • Case manage patients as needed.
  • Oversee regulatory/accreditation compliance.
  • Review and update policies and procedures as appropriate.
  • Lead interdisciplinary group meetings and case conferences.
  • Oversee per diem staff scheduling.
  • Identify competency and staff development needs; ensure compliance.
  • Attend seminars/workshops as appropriate.
  • Cover Management Back-Up and at least one holiday per year.

Job description

This is a full-time professional position. This position is responsible for coordinating nursing staff to ensure day-to-day continuity of care for all patients on service and ensure accountability of nursing care consistent with the agency’s mission statement and regulatory systems. This position supervises Transitions Community-based Hospice nurses plus day to day supervision of all interdisciplinary team members.The schedule for this position is Monday-Friday 8:00am-5:00pm; on-call rotation; one major and one minor or three minor holidays per year.

Responsibilities

  • Coordinate interviewing/hiring of new clinical staff as assigned.
  • Supervise and coordinate delivery of patient care for assigned patient care area on a 24-hour/day basis.
  • Coordinate and problem-solve patient care issues with patients, families, physicians, agency staff and other health care providers.
  • Ensure documentation of patient care is complete, timely, and accurate in accordance with TL processes, policies, procedures and regulatory requirements.
  • Manage various personnel actions including, but not limited to hiring, orientation, performance appraisals, promotions and scheduling; and counsel and conduct formal discipline of clinical staff.
  • Develop and implement processes to enhance clinical operations and practices for ongoing quality improvement.
  • Case manage patients, as needed/requested.
  • Oversee clinical compliance with all regulatory and accreditation needs of the agency.
  • Review, update, and implement policies and procedures as appropriate, per agency protocol.
  • Lead interdisciplinary group (IDG) meetings and case conferences.
  • Oversee preparation of per diem staff calendar/scheduling.
  • Identify annual competency and staff development needs, in conjunction with PI, Education, and Managers, ensuring all clinical staff are compliant with identified competencies.
  • Attend seminars/workshops as appropriate.
  • Covers Management Back-Up (MBU) as assigned.
  • Covers at least one holiday per year as assigned.
  • Professionally manages time, people, and technology.
  • Perform other duties as assigned by supervisor.

  • Adhere to all TL policies, procedures, and organizational guidelines, ensuring compliance in daily work and decision-making.

  • Promptly communicate concerns, issues, or potential risks to your immediate supervisor or another member of the management team to support a safe and effective work environment
Requirements

  • RN with current N.C. license.
  • Two years of previous supervisory experience preferred.
  • 5 years recent clinical experience with hospice, home health, or equivalent.
  • Thorough understanding of Hospice clinical interventions and protocols.
  • Knowledgeable about reimbursement sources, Medicare/Medicaid regulations, licensing laws and accreditation standards.
  • Excellent written and verbal communication skills.
  • Strong team building skills

  • Competent in use of computer, able to learn and teach use of new software programs.
  • Must have access to transportation.
  • Requires travel to TL offices within NC.
  • May be required to lift 30 lbs.
  • Computer literacy with Windows programs. Ability to learn Netsmart programs.
  • Current professional license or certification.
  • Current CPR certification.
