Shift options include day, night, and mid shifts. Job types available are full-time, part-time, and per diem. Qualifications
  • Bachelor of Science in Nursing (BSN) or higher from an accredited program
  • Active and unencumbered RN license
  • At least 1 year of case management experience in healthcare or hospital settings
  • Exceptional communication skills, both written and verbal
  • Proficiency in EHR software
Responsibilities
  • Conduct comprehensive patient assessments covering medical history, medications, treatment plans, and psychosocial needs
  • Collaborate with healthcare teams and patients to develop and implement care plans
  • Monitor patient progress and adjust care plans accordingly, communicating updates to the healthcare team
  • Act as the primary liaison between patients and healthcare providers to ensure seamless communication
  • Maintain accurate and confidential case records in compliance with standards
