General Description:

As a member of a multidisciplinary care management team, the nursing case manager is accountable for coordinating the care and services of selected patient populations.

Essential Responsibilities:

• Assess patient goals comprehensively, including biophysical, psychosocial, environmental, discharge planning needs, and financial status.
• Monitor patient progress closely for changes in medical needs that could affect treatment plans.
• Procure services and advocate for patients and families with limited resources.
• Collaborate with third-party payers to resolve issues affecting continuity of care.
• Coordinate and direct discharge planning with patients, families, healthcare team, and community resources.
• Review admission data within designated timeframes to assess the appropriateness of care levels according to ISD-A criteria or established standards.

General Responsibilities:

• Perform other duties as assigned.

Minimum Qualifications:

Education: Graduate from an accredited nursing school is required. A Bachelor of Science in Nursing or higher is preferred.

Experience: Minimum of 3 years clinical nursing experience in an acute care setting.

Licenses/Certifications: Current RN license issued by the Oklahoma State Board of Nursing or a multistate compact RN license (eNLC). Current Basic Life Support certification from the American Heart Association is required upon hire.

Knowledge, Skills, and Abilities:

• Knowledge of nursing practices and procedures.
• Strong clinical assessment skills.
• Knowledge of community resources to facilitate patient recovery.
• Understanding of third-party payer issues related to reimbursement and regulations.
• Ability to explain complex information clearly to patients and families.
• Strong interpersonal skills for effective information gathering and assessment.
