Job summary

The Kenaitze Indian Tribe is seeking a per-diem Nurse Case Manager to assess, plan, implement, and evaluate health care for assigned panels. This role requires advanced knowledge in health care management and provides critical coordination for comprehensive care. Candidates must hold an RN license and have significant clinical experience, alongside the ability to support culturally diverse patients in their health journeys.

Qualifications

• Three (3) years of clinical experience required.
• Experience with patient population health management.
• RN license required.

Responsibilities

• Evaluate processes for targeting health status of assigned panels.
• Coordinate and oversee health care service processes.
• Advocate for and assist patients in navigating healthcare systems.
