Job summary

Sentara Healthcare in Northern Virginia is seeking an Integrated Nurse Case Manager. This full-time role requires travel for face-to-face home visits across the Northern Virginia region, including Arlington and Manassas.

Primary duties include developing and monitoring member care plans and coordinating with the care team to optimize health outcomes. The ideal candidate has an RN license (compact/multi-state), 3 years of nursing experience, and exposure to discharge planning and managed care.

Qualifications

• Associates or Bachelors Degree in Nursing required.
• RN License - Compact/Multi-State License required.
• 3 years nursing experience required.
• Discharge planning experience preferred.
• Managed Care experience preferred.

Responsibilities

• Provide case management services within licensure scope; develop and monitor care plans.
• Conduct telephonic or face-to-face assessments to identify and coordinate member needs.
• Identify high-risk members and coordinate care with the health care team.
• Manage chronic illnesses and disabilities to ensure cost-effective utilization of benefits.
• Assist with implementation of care plans by facilitating authorizations/referrals within benefits structure.
• Present cases at case conferences for multidisciplinary focus.

Compensation within the range may vary based on qualifications, experience, location, market conditions, and business needs.
