Job summary

Nantucket Cottage Hospital is seeking a per diem Registered Nurse to join the care team at our campus in Nantucket. You will coordinate patient care, support discharge planning, and collaborate with providers and families to ensure seamless transitions.

Provides outreach and enrollment services to meet eligibility requirements of the program and surrounding counties. Ensures coordination and planning, including community and family support. Focuses on the development and coordination of community service plans.
