Job summary

Qualifications

• At least one full year of professional nursing experience at or above the GS-9 level with a nursing degree.
• Ability to develop care coordination and management plans across the continuum.

Responsibilities

• Serve as clinical liaison for staff, clarifying referrals and coordinating care.
• Develop care plans with patient/family and interdisciplinary teams.
• Prepare for discharge and arrange ongoing support with tribal or external providers.
• Perform intake screening and biopsychosocial assessments to guide service placement.
• Document findings and apply nursing process in patient care.
