Job summary

Qualifications

Responsibilities

• Manage complex chronic, acute, and multi-system caseloads with assessment, care planning, discharge coordination, follow-up, education, and advocacy.
• Coordinate multidisciplinary care, referrals, authorizations, transitions, and follow-up; ensure timely, continuous care.
• Provide outpatient nursing care through in-person and telehealth encounters, including assessment, treatment, and safety monitoring.
