Job summary

Elevance Health in Indianapolis is seeking a Nurse Case Manager II to perform care management for members with complex health needs. You will assess, develop, implement, coordinate, monitor, and evaluate care plans, often telephonically or on-site for discharge planning.

The role requires a BA/BS in a health-related field, an active RN license, and multi-state licensure if applicable. Hybrid/virtual work with in-person onboarding is supported.

Qualifications

• Requires BA/BS in a health related field and minimum of 5 years of clinical experience; RN license in applicable state(s) required; multi-state licensure required if providing services in multiple states.

Responsibilities

• Ensures member access to services appropriate to health needs.
• Conducts assessments to identify needs and develop care management plans.
• Implements care plans by facilitating authorizations/referrals within benefits or through extra-contractual arrangements.
• Coordinates internal and external resources to meet identified needs.
• Monitors and evaluates the care management plan and updates as needed.
• Interfaces with Medical Directors and Physician Advisors on care plans.
• Negotiate rates of reimbursement, as applicable.
• Assist in problem solving with providers, claims or service issues.
• Assist with development of utilization/care management policies and procedures.
