Job summary

Elevance Health in Indianapolis seeks a Nurse Case Manager II to manage complex, chronic cases by assessing, developing, coordinating, and monitoring care plans across the continuum. The role can be hybrid with required in-person training and occasional on-site work.

You will collaborate with medical directors, coordinate authorizations, and monitor plan effectiveness while ensuring access to services and timely discharge planning.

Qualifications

• Requires BA/BS in a health field and at least 5 years of clinical experience.
• Current, unrestricted RN license; multi-state licensure required.
• Preferred CM certification and BS in health/human services.

Responsibilities

• Performs discharge planning telephonically or on-site.
• Ensures member access to services appropriate to health needs.
• Conducts assessments to identify needs and develops care plans.
• Coordinates authorizations/referrals within benefits or extra-contractual arrangements.
• Coordinates internal and external resources to meet identified needs.
• Monitors and evaluates care plan effectiveness; interfaces with Medical Directors.
• Assists with problem solving with providers, claims, or service issues.
• Contributes to utilization/care management policies and procedures.
