Qualifications

• RN licensure with state validity (New Jersey).
• Reliable transportation to complete face-to-face visits; mileage reimbursed by policy.

Responsibilities

• Monitor and evaluate care plans and adjust based on clinical judgement and member needs.
• Coordinate with members, care team, and providers to ensure timely service delivery.
• Advocate for member needs and assist with safe discharge and post-hospitalization transitions; document in EHR.
