Geisinger at Home nurses deliver personalized healthcare to Medicare members in patients’ homes, serving Columbia, Montour, and Northumberland counties. The role collaborates with physicians and a multidisciplinary team to develop and adjust care plans, ensuring progress toward expected outcomes.

Typical hours are Monday through Friday, 8:00 AM–4:30 PM, with one 10:00 AM–6:30 PM shift weekly, plus rotating weekends and holidays.

Qualifikationen

• RN with 3+ years of experience required
• Experience in case management, critical care, emergency, or home healthcare is a plus
• Graduate from Specialty Training Program-Nursing (Required)
• Bachelor's Degree-Nursing (Preferred)

Aufgaben

• Develop and manage plan of care for each patient from admission through discharge
• Coordinate efforts of physicians, nurses, and other team members to ensure quality care
• Assess patient and family needs and respond to barriers to care
• Facilitate safe and timely discharge planning and transition to community services
