Job summary

Boise VA Medical Center is seeking an experienced RN to serve as Case Manager in the Home Based Primary Care program. You will manage care across the health care continuum, provide assessments, educate patients and families, and coordinate services to ensure timely, quality care in the home setting.

The role requires active RN licensure in the United States, graduation from an ACEN/CCNE accredited program, and strong English communication skills.

Qualifications

• English language proficiency required for direct patient care.
• Graduate of a nursing program accredited by ACEN or CCNE.
• RN licensure in the United States with active status.
• Bridge programs or BSN options may prepare for licensure.
• Documentation and licensing requirements as per VA standards.

Responsibilities

• Serve as Case Manager for Home Based Primary Care (HBPC) patients.
• Coordinate and plan nursing care across the health care continuum.
• Provide assessments, teaching, and caregiver support in the home.
• Monitor patient condition and facilitate timely referrals.
• Collaborate with the healthcare team to optimize outcomes.
