University Health in Texas is seeking a Nurse Case Manager to coordinate the treatment plan for patients during acute care episodes. You will work with patients and the interdisciplinary team to ensure timely service delivery and advocate for patient needs.

The role requires a BSN and 2–5 years of nursing experience, with preferred experience in case management, utilization review, or hospital quality. Texas RN licensure and CM certification are advantages.

Qualifications

• Bachelor's degree in Nursing preferred.
• Two to five years nursing experience required.
• Experience in case management, utilization review or hospital quality preferred.

Responsabilités

• Coordinate the treatment/plan of care for patients during the acute episode of care.
• Proactively facilitate interventions to ensure timely delivery of services.
• Evaluate the effectiveness of interventions and track variances or barriers in the plan of care.
• Advocate for the patient and communicate health care needs to the interdisciplinary team.
