Qualifications

• Diploma/AD required.
• BSN preferred.
• Active RN license in the applicable state(s).
• Case management certification preferred.
• Minimum of five years of progressive clinical experience, including utilization management or case management.

Responsibilities

• Assesses inpatient and ED discharge planning needs and social drivers of health.
• Collaborates with providers, social work and the team to develop patient-centered plans of care and discharge plans.
• Monitors clinical progression and barriers to discharge and escalates as needed.
• Partners with Utilization Management to support medical necessity and correct patient status.
• Identifies patients at risk for unsafe transitions and readmission; coordinates follow-up care.
• Communicates with patients/families and care team to address barriers and adapt plans.
• Provides patient/family education on discharge and follow-up needs.
• Initiates referrals to internal and community resources to support discharge needs.
• Coordinates transitions of care with outside hospital/facility case management.
