Qualifications

• Current RN license in Wisconsin.
• Inpatient case management experience preferred.

Responsibilities

• Conduct comprehensive assessments and create timely care plans & interventions.
• Collaborate with patients, families, and multidisciplinary team to meet goals.
• Provide continuity of care and discharge planning aligned with regulatory standards.
• Coordinate transfers and handoffs across units and settings.
• Deliver case management services using community resources.
• Initiate referrals and document discharge planning and UM activities.
