Job Summary

The Nurse Case Manager will be responsible for scheduling caregivers and ensuring effective care coordination for a caseload of clients.

Role and Responsibilities

• Be the champion of change for caregivers, ensuring they feel valued and understand their importance to Family Tree.
• Develop lasting relationships with caregivers, ensuring they are well-prepared and drive client satisfaction.
• Ensure client-caregiver matches are appropriate by scheduling skilled personnel suited to client needs.
• Mentor, coach, and reward caregivers to reach their full potential.
• Attend weekly meetings with supervisors and care coordinators to discuss current caseloads and staffing needs.
• Review care plans for new clients and update schedules in ClearCare to monitor staffing progress.
• Negotiate pay rates within established parameters based on case difficulty, location, and urgency.
• Assign pay rates and bonuses to caregivers (with approval) within company policies.
• Perform regular check-ins with caregivers to set expectations regarding conduct, dress, and timeliness.
• Document caregiver check-ins and escalate concerns as needed, ensuring proper communication and documentation in ClearCare.
• Collaborate with division supervisors on staffing performance, punctuality, and reliability.

Requirements

Qualifications and Education

• High school diploma required.
• Minimum 2 years of scheduling experience in home care required.
• Experience with ClearCare preferred.

• Thorough understanding of the home care business.

Physical Requirements

• Prolonged periods of sitting at a desk and working on a computer.
