Description

The Nurse Case Manager is responsible for planning, coordination, implementation and evaluation of health care and

social services to participants at the PACE program. Serves as the liaison between the interdisciplinary team (IDT),

participants, caregivers and community agencies.

Position Purpose

Join a mission-driven team at Trinity Health PACE, where you’ll lead and coordinate high-quality nursing care for participants in our Program of All-Inclusive Care for the Elderly (PACE). Care may be provided in our day center or in participants' homes.

What You Will Do

• Lead, support, and mentor nursing staff across settings
• Collaborate with interdisciplinary teams to develop and oversee care plans
• Monitor clinical outcomes, safety, and regulatory compliance
• Oversee budgets, supplies, staffing, and training
• Coordinate care with outside providers (hospitals, nursing facilities, etc.)
• Participate in quality improvement, compliance audits, and organizational planning
• Serve on-call and support weekend staff as needed
  
  

Minimum Qualifications

• Current RN license and BSN required
• 3+ years of healthcare experience, including at least 1 year working with elderly or frail populations
• Prior supervisory or leadership experience
• Valid driver’s license and CPR certification
• Strong leadership, communication, and organizational skills
• Proficiency in Microsoft Office and electronic health records
