Mentor Community Services - Nursing Case Manager (Full Time) - IDD Services - LPN or RN

Office Location: Canton

Coverage Areas: Brockton/Randolph/Abington/New Bedford/Fall River/Avon... AND ... Randolph/Mattapan/Dorchester/Hyde Park/Boston/Roslindale/Jamaica Plain

Position Type: Hybrid (flexible schedule)

Salary Range: $65,000 - $72,000

Job Description

Daily responsibilities include coordinating client discharges, conducting intake assessments for new clients, auditing in-home medication records, managing new medication orders, and training staff on new diagnoses. The ideal candidate should have a solid understanding of basic nursing concepts, including pharmacology, and be able to manage multiple tasks efficiently in a fast-paced environment. Excellent communication and organizational skills are essential, along with a proactive mindset and a desire to learn and grow professionally. Basic computer skills (email, Excel, Word) are beneficial. Knowledge of 6400 regulations is a plus, but training will be provided. Candidates should be self-motivated with strong time management skills.

Qualifications
  • Valid state nursing license (LPN or RN)
  • Minimum 1 year of relevant experience
  • Valid driver’s license and vehicle (1+ year)
  • Detail-oriented, with the ability to multitask
  • Excellent communication and analytical skills
