Job Summary:

Hours: Monday - Friday, 8:00 am-5:30 pm - full-time or part-time schedule possible

We are seeking an HIV Nurse Case Manager to work on a multidisciplinary HIV primary care team recognized as a model of excellence. You will collaborate with medical providers, social workers, behavioral health team members, and other HIV nurses to provide high-quality nursing care aligned with patient-centered, non-judgemental, harm reduction, and team-based principles. Your primary responsibilities include HIV and chronic disease management, care coordination, patient advice/triage, and ensuring clear communication within the care team.

You will conduct clinical nursing assessments and triage during outpatient visits, outreach to shelters, drop-in centers, street venues, and patients’ homes. You will also support medication adherence, health education, and behavioral risk reduction, promoting linkage and retention in HIV care to help patients achieve and maintain health. This position is funded until March 2028, with potential for renewal. If funding is not renewed, employment will end on March 31, 2028.

Responsibilities:

  • Provide outpatient primary care nursing, including assessments and triage for HIV patients during routine and urgent visits, via virtual, telephonic, or in-person methods. Document accurately in the EHR.
  • Implement and evaluate the patient-centered care plan, communicate progress, and conduct intake assessments for new patients.
  • Administer vaccines, injectable medications, and perform wound care; assess medication adherence and provide counseling; support directly observed therapy when needed.
  • Coordinate care with behavioral health staff, visit outreach sites, and identify barriers to care and medication adherence.
  • Support management of HIV and other chronic conditions, addressing structural barriers like housing, transportation, and benefits.
  • Assist with follow-up on medications for substance use disorders and coordinate preventive services including education, vaccinations, testing, and referrals.
  • Foster a compassionate, therapeutic environment, participate in training, and contribute to service improvement initiatives.
  • Establish respectful relationships with patients and collaborate effectively with healthcare team members and external agencies.

Qualifications:

  • Registered Nurse license in Massachusetts
  • Bachelor’s degree in nursing preferred; minimum 2 years outpatient care experience required. HIV care experience strongly preferred.
  • ACRN certification preferred; support for training available.
  • Knowledge of substance use disorder and harm reduction principles preferred.
  • Experience with homeless populations and trauma-informed care is a plus.
  • Community health center experience is a plus.

Compensation and Benefits:

  • Starting at $40/hour, commensurate with experience
  • Full-time employees are eligible for 4 weeks’ vacation, health/dental/vision insurance, 403B retirement plan, employer contribution, and discounted MBTA passes. Additional benefits include yearly increases, bilingual bonuses, and opportunities for collaboration with hospitals and community centers.
