Job summary

Discovery Senior Living in Texas is seeking a licensed nurse to lead the clinical team and ensure compliant, high-quality care for residents. The role focuses on supervising daily operations and collaborating to maintain service plans in line with state requirements.

The position requires active LVN/LPN licensure, leadership in clinical settings, and strong communication with families and staff. On-call rotations and staff training are included as part of the duties.

Qualifications

• Current LVN/LPN license in good standing.
• Experience leading clinical care in an assisted living or similar setting.

Responsibilities

• Ensures community follows all federal, state, local laws and regulations as it pertains to clinical care.
• Identifies ongoing needs and services of residents through the assessment/Service Plan process including documentation for residents with a change in health care status.
• Partners with other department coordinators to identify changes in service/medication levels and updates the resident’s service plan as indicated by state regulations.
• Maintains communications with resident’s family and/or responsible party regarding changes in care or health concerns.
• Carries out supervisory responsibilities including interviewing, hiring, and training employees; planning, assigning, and directing work; addressing complaints and resolving problems.
• Manages the day-to-day healthcare operations of the community to ensure residents' healthcare needs are met.
• Assists in the supervision of medication technicians, caregivers and provides training to care staff on an ongoing basis.
• Shares on call duties, 6-week rotation
