Job summary

PrideGlobal is seeking an experienced Registered Nurse Care Manager for a hybrid role in the Bronx, NY. This full-time position blends field visits with remote care coordination to support older adults and individuals with complex healthcare needs.

Responsibilities include in-home assessments, care plan development, cross-disciplinary collaboration, and coordinating care across home care, hospitals, and long-term care settings. NY RN license required.

Qualifications

• Active, unrestricted New York RN license.
• Associate’s degree in nursing required; BSN preferred.
• Minimum 3 years of RN experience.
• Experience in MLTC, home care, geriatrics, or case management preferred.
• Strong communication, organizational, assessment, and critical‑thinking skills.
• Ability to work independently in a hybrid environment.
• Bilingual skills are a plus.
• Field Schedule: Approximately 6 home visits per day.
• Training: Onsite at Bronx office.
• Salary: $100,000–$130,000 per year.

Position Overview

We are seeking an experienced Registered Nurse (RN) Care Manager for a hybrid position supporting older adults and individuals with complex healthcare needs. This role combines community-based field visits with remote care coordination, offering flexibility while making a meaningful impact on members’ health and quality of life.

Responsibilities

• Conduct in-home assessments to evaluate members’ clinical, functional, and social needs

• Complete approximately 6 home visits per week within the Bronx territory
• Monitor member progress and adjust care plans to support optimal outcomes

• Communicate effectively with members, families, caregivers, and healthcare providers
• Maintain accurate and timely clinical documentation in accordance with applicable guidelines
• Advocate for members and connect them with appropriate healthcare and community resources
