Qualifications

Responsibilities

• Acts as expert clinical practitioner within the Case Management team and mentors CM staff.
• Assesses patients' needs and coordinates with physicians and care teams.
• Coordinates social services to address patient and family needs.
• Manages discharge planning and follow-up to ensure continuity of care.
• Monitors quality, variances and utilization to improve patient outcomes.
• Uses CM information systems to track progress and develop action plans.
• Promotes patient and family satisfaction and communicates ICARE principles.
