Qualifications

• Five years of nursing experience required.
• Experienced in Case Management, Home Health, Medical/Surgical Nursing, Emergency Room or Ambulatory Care.

Responsibilities

• Coordinates care across the healthcare continuum with the care team.
• Educates and informs patients, families, and caregivers about treatment plans.
• Maintains a registry of high‑risk patients and tracks measures and interventions.
• Utilizes EMR and tracking tools to optimize workflows and documentation.
• Collaborates with providers and community partners to access services and resources.
• Supports patient transitions between facilities and care settings to ensure continuity.
