United Services is looking to hire a full-time Nurse Care Manager (LPN/RN) for our Behavioral Health Home. If you are passionate about working with individuals with behavioral health diagnoses and chronic health conditions with a team approach and are looking for a supportive work environment that encourages a good work-life balance, we are looking for you! The Nurse Care Manager works Monday-Friday 8:30am-5pm with no evening, weekend, or holidays required. This is a great opportunity to work with individuals to meet their health and wellness goals, provide person-centered care, and provide care coordination to have a positive impact on one’s life.
The Nurse Care Manager will provide care management, care coordination, individual and family support, hospital transition planning, and health promotion services to Behavioral Health Home enrollees and Comprehensive Services clients.
Minimum Requirements:
Current Registered Nurse (RN) / Licensed Practical Nurse (LPN) license. Able to maintain and renew licensure as RN/LPN in the state of Connecticut.
Monday – Friday 8:30am – 5:00pm
