Job summary

Human Coalition in Grapevine, TX is seeking a Nurse Care Coordinator (RN/LVN) to provide face-to-face in clinic and telecare services. The nurse delivers non-diagnostic telephone and in-person assessment, education, and support to clients facing unplanned pregnancy, supporting mission to serve families across America.

This full-time, onsite role reports to the Practice Manager and requires licensure, CPR, and 2–4 years of clinical experience; obstetric/gynecologic care is preferred.

Qualifications

• Must hold a current compact LVN or RN nursing license.
• 2–4 years of prior clinical experience in any specialty.
• Preference for obstetrical and gynecological care.
• Must have current and up to date CPR certification healthcare providers.

Responsibilities

• Effectively evaluate a client’s situation and address concerns over the phone.
• Provide education on abortion, parenting, and adoption services to patients.
• Connect women with necessary resources to relieve pressure.
• Utilize knowledge base to provide quality follow up care to clients.
• Perform limited medical services to women facing unplanned pregnancies.
• Be a key member of the medical team onsite providing compassionate care.
• Offer counseling, education, and emotional support to clients and families.
• Document all client interactions and execute care plans in CMS.
• Notify Medical Director with pertinent information as per policy.
• Interact with patients with empathy while adhering to scope of practice.
