Job summary
Family Health Center is seeking a Home Health Registered Nurse to join the Primary Care Team in Worcester, MA. This regular full-time role coordinates home health, quality improvement, and utilization review for health center patients, ensuring timely and appropriate care within policy guidelines.
The candidate should hold an RN license in Massachusetts, a Bachelor's in Nursing, and ideally have home health experience.
Qualifications
- Bachelor's degree in nursing required.
- Must be licensed as an RN by the Massachusetts Board of Registration. BLS required.
- Home health experience preferred.
Responsibilities
- Coordinate home health, quality improvement, and utilization review for patients.
- Communicate determinations to providers and home care agencies in writing per policies.
- Review home health agency communications and provide documentation as needed.
- Assist with DME requests.
- Refer high-risk cases to Care Management and integrate QI activities.
