Job summary

NHC HealthCare Lexington is seeking a Nurse Assessment Coordinator (RN or LPN) to oversee patient assessments and ensure accuracy of MDS coding in a geriatrics-focused setting.

The role requires strong organizational skills, collaboration with an interdisciplinary team, and adherence to Medicare/Medicaid rules. Competitive pay and a comprehensive benefits package are offered.

Qualifications

• RN or LPN license in good standing.
• CMAC certification preferred.
• Professional experience in geriatrics nursing.
• Knowledge of Resident Assessment Instrument and applicable rules/regulations.

Responsibilities

• Organizational skills, ability to schedule and meet deadlines.
• Monitor overall medical records to validate support of MDS coding.
• Work with interdisciplinary team to assure timely completion of Care Area Triggers and Patient Care Plans.
• Support the accuracy of medical record documentation through ongoing in-services to center partners.

Qualifications

• Registered Nurse/Licensed Practical Nurse – graduate of an accredited school of nursing and currently licensed to practice nursing in the state.
• CMAC Certification preferred.
• Professional experience in the practice of geriatrics nursing.
• Knowledge and expertise in the completion of the Resident Assessment Instrument including all applicable rules and regulations.
• Knowledge of SNF Part A Medicare and Consolidated billing rules.
