• Assist residents with activities of daily living per care plan.
• Report changes to nursing staff and document vitals as required.
• Support mobility, bathing, feeding, dressing, and personal hygiene tasks.
• Observe and report symptoms or changes in residents’ conditions.
• Maintain cleanliness and safety of resident rooms and equipment.
• Document care activities in Electronic Medical Records or logs.
• Respond promptly to call lights and requests for assistance.
• Provide compassionate, respectful care and maintain resident privacy.
