Summary

The Certified Nurse Aide I is responsible for personal care and related services to home health, hospice, and home care patients as outlined in the patient's individualized plan of care, delegated by the qualified professional.

CNA Essential Job Functions

1. Creates positive experiences for internal and external customers that meet their expectations. Displays courtesy, tact, and knowledge of services provided by the agency in all interactions with staff, patients, and visitors.
2. Administers personal care and hygiene following the patient's individualized plan of care.
3. Accurately takes and records vital signs and weight, as ordered. Reports and records changes in the patient's mental or physical condition immediately to RN or Clinical/Assistant Director to prevent adverse events.
4. Participates in the orientation of new staff to patients, family members, and the home setting as requested.
5. Submits documentation using the MobileCare system daily to verify schedules and care plans; enters progress notes as per policy; enters employee time accurately and timely; submits weekly time and travel sheets; and provides other data to supervisors promptly.

Requirements for Hospice Nursing Assistant

1. Listed as a CNA with DHSR, with successful training completion and no lapse of more than 24 months without providing paid services.
2. Current CPR certification.
3. Ability to read and write.
4. Dependable and capable of dealing pleasantly and tactfully with patients, families, staff, and the public.
5. Valid North Carolina Driver's License and transportation.
